Such a template does exist. I wrote one in LinuxDoc DTD, available as
	SGML http://www.nyx.net/~sgjoen/template.sgml
	HTML http://www.nyx.net/~sgjoen/template.html
and it has also been translated into DocBook DTD. I wrote it to be
included or at least referenced from the author's section of the
LinuxDoc site. BTW it was updated recently with an updated copyright.

The Template can be used for that, some guidelines are in <em></em>
with sample text in plain font. Additionally the SGML is commented
to show you how you go about producing links, tables and more. It
was made by extracting the structure of my Multi Disk HOWTO

so it is based on a practical case of a large HOWTO.
